Facility Evaluation Report
On 12/04/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Carlos Peoples as the purpose of the visit was explained. The facility is licensed to serve 4 developmentally disabled adults ages 18-59 of which 1 may be non-ambulatory. Bedroom #1 approved for non-ambulatory. Clients are linked to the Harbor Regional Center. Facility fees are current.
The facility is a single-story structure in a residential neighborhood. The facility consists of (4) bedrooms, (2) full bathrooms, (1) half bathroom, living room, sitting room, dining room, kitchen, laundry room, detached garage, and outside shaded patio area. LPA observed a shaded patio area with table and chairs available. LPA observed all walkways were clean, clear, and free of debris, hazards, and obstructions. Gates exiting the property open easily from the inside to exit onto the street. Handrails along outside ramps and stairs are secured. There are no bodies of water on the premises. A land line and internet was observed. Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to clients.
LPA conducted a records review of 5 staff records, 4 client records, 4 P&I ledgers, and 4 medication administration records, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 11/09/25, 3 fire extinguisher fully charged, carbon monoxide and smoke detectors were observed to be operational.
