The inspector’s account

On 11/04/25 Licensing Program Analysts (LPAs) Villegas and Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Elaine D. Williams as the purpose of today’s visit was explained. The facility is licensed to serve 6 developmentally disabled clients (age 18-59). Facility fees are current.

The facility is a single-story structure located in a residential neighborhood and consists of the following: The home consists of 3 Bedrooms, 2 private bathrooms, 1 shared bathroom, a linen closet, living room, dining area, kitchen, a stocked pantry, an outdoor shaded patio area with a washer and dryer, an attached garage that serves as storage unit and houses an additional refrigerator and freezer.

All client rooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Toxins and knifes were observed to be locked and inaccessible to clients. Exits/ Walkways around the facility were free of debris and hazards.

LPA conducted a records review of 4 staff records, 6 client records, and 6 medication Administration Records. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire drill was conducted on 06/07/25, 2 fire extinguishers fully charged, carbon monoxide detectors and smoke detectors are operational. Land line and internet service was observed.

Exit interview conducted, appeal rights explained, and a copy of the report was provided.
