Facility Evaluation Report

On 11/13/2024 The Department of Social Services, Community Care Licensing Division (CCLD) staff conducted a case management visit at the above-mentioned facility. CCLD staff was met by Rebecca Flores, program manager (S1), and the purpose of the visit was explained. CCLD staff and S1 toured the facility and the facility was observed as clean and in good repair during the visit.

There have been zero (0) deficiencies cited during today's visit.
