Facility Evaluation Report

On 09/30/2025 at 8:25am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Marilee "Mary" Cruz, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (20) ambulatory, (0) non-ambulatory, (0) bedridden developmentally disabled adults ages 18 through 59. Currently, the home has (18) clients. None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a current administrator certificate (#7033562735) for Marilee Cruz valid from 10/10/202 - 10/09/2027. The facility fees are current. The facility has liability insurance with Old Republic Insurance Company (NAIC# 24147) with each occurrence at $2,000,000 and general aggregate at $4,000,000 (policy #MWZY313733-25) effective date 10/01/2025 - 10/01/2026.

The facility is a one story home located in a residential neighborhood. The property consists of the following: 10 resident bedrooms, 4 common bathrooms, staff office, living room, storage room, kitchen, pantry room, dining room, a linen closet, a laundry room with a washer and a dryer with additional linen, and backyard and an outdoor shaded area in the front of the facility.

Between the hours of 9:15am - 10:33am, LPA conducted a records review of (8) client records, (4) staff records, (6) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) Client Medication Administration Records and did not observed any discrepancies at the time of visit.
