The inspector’s account

On 12/11/2023 at around 8:50 AM, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with the Administrator Marilee Cruz. LPA explained the purpose of the visit and was accompanied by Administrator inside and outside the facility during this inspection.

This facility is licensed to serve 42 ambulatory mentally ill adults ages 18 – 59 years.

The facility has an approved waiver for incidental medical services provided for clients who have insulin dependent diabetes.

A total of 22 ambulatory clients are currently residing in this facility.

The facility is composed of two buildings in a residential street.

Building 1 is a two-story building that consist of 19 client bedrooms, 6 bathrooms, 1 laundry room, 1 kitchen, 1 staff room, 1 kitchen storage, and 1 file room.

Building 2 is a one-story building that consist of 6 client bedrooms, 2 client toilets, 2 client showers, 1 staff bathroom, 2 offices, 2 storage rooms, 1 tv room, 1 conference room, and 1 dinning room.

There is a backyard patio area with shaded seating.

Outside grounds were toured and no bodies of water were observed. The patio furniture is under a shaded area and accessible to clients. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises.
