Facility Evaluation Report

On 04/16/2025 at 8:17 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At 8:47am, LPA met with the Maria Cristina Serafico, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate (3) non-ambulatory, adults only. Currently, the home has (3) clients. The clients are Harbor Regional Center clients. Two (2) the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices .

The facility has a current administrator certificate (#7028306735) for Maria Cristina Serafico valid for 08/04/2024 - 08/03/2025. The facility fees are a balance of $454 due on 06.04.2025. LPA provided pin #328771 with the option for the facility to make an payment online. The facility has liability insurance with National Union Fire Ins Co of Pittsburgh (NAIC# 19445) with each occurrence/general aggregate at $4,000,000 - (policy #3752210) effective date 10/01/2024 - 10/01/2025.

The facility is a one (1) story home located in a residential neighborhood. The property consists of the following: three (3) client bedrooms, two (2) bathrooms, a living room, a kitchen, a dining room, an office area, a den area, an attached garage and a outdoor shaded area.

Between the hours of 9:35am - 11:00am LPA conducted a records review of (3) client records, (5) staff records, (3) clients Personal & Incidental Records, (3) Client Medication Administration Records and the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current (last conducted on 03.02.2025) and in compliance with Title 22 at the time of visit. LPA did not observed any discrepancies at the time of visit.
