Facility Evaluation Report

On 09/29/2025 at 12:34 pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Theresa Guzman and the purpose of today’s visit was explained. The facility is licensed to operate for (2) ambulatory, and (1) non-ambulatory, who are developmentally disabled adults ages 18 through 59. Currently, the home has (3) clients. The clients are Harbor Regional Center clients.None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a current administrator certificate (# 7002906735 ) for Ma Theresa Guzman (Administrator) valid from 02/08/2024 -02/05/2026 . The facility has liability insurance with Philadelphia Manufactures Association Insurance Co (NAIC# 12262) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #822501 1530427) effective date 04/17/2025 - 04/17/2026. The facility fees are current.

The facility is a one story home located in a residential neighborhood. The property consists of the following: five (5) bedrooms (with 2 of the room being client bedrooms), 2 common bathrooms, staff office, living room, kitchen (with water temperate at 106.2 °F ), dining room, attached garage which houses an additional refrigerator & can good and an outdoor shaded area in the backyard.
