Facility Evaluation Report
On 05/12/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced case management visit to issue citation observed during complaint investigation # 11-AS-20250418141748. LPA met with Assistant Administrator Cathrine Dacara as the purpose of the visit was explained. LPA noted during complaint investigation on 05/12/25 the facility failed to report to CCLD that R1 was placed on a 5150 hold on 3/25/25. During file review, facility could not provide proof/a fax confirmation that the incident report was sent to CCLD within seven days of the occurrence.
Exit interview conducted, appeal rights explained, and a copy of the report was provided.
