The inspector’s account

On August 21, 2025, at 1:00 PM, an office meeting was held to discuss Complaint #11-AS-20220120114542. Present at the meeting were Janae Hammond, Licensing Program Manager (LPM); Zina Brown, Licensing Program Analyst (LPA); Kathrina Buller , Licensee Representative/Administrator.

During the meeting, the LPM reviewed the details of the complaint. On March 16, 2024, the Department substantiated an allegation of neglect and lack of care and supervision, which resulted in a resident developing pressure injuries.

At the time the findings were delivered on March 16, 2024, the Department indicated that an enhanced civil penalty determination was pending, pursuant to Health and Safety Code Section 1569.49(e), relating to Serious Bodily Injury.

The Department is reviewing the complaint for enhanced civil penalty for serious bodily injury pursuant to H&S 1569.49(e). The total amount for the civil penalty totals $10,000 for Serious bodily injury.
