Facility Evaluation Report

On July 28, 2025, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met Amalia Ordoveza, DSP who granted access to facility. Subsequently, LPA was joined by Danny Tapia, Director to which LPA explained the purpose of today’s visit. The facility is licensed to operate for (3) non-ambulatory Adults adults age range 18 through 59. of which one my be bedridden. The residents are all Harbor Regional Center consumers.

The facility is a single-story building located in a residential neighborhood. It consists of the following: (3) client's rooms, (2) bathrooms, a living area, a dining area, a kitchen, an outside seating area, and a garage used for storage.

LPA inspected all 3 bedrooms; the mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked.
