Complaint Investigation Report
Record Review of CA Mandt Training revealed nine out of nine staff members who worked on 03/26/26 PM received training. Four out of four staff interviews (S2 – S5) denied the allegation. S2, S4, and S5 indicated staff is trained in MANDT and use an open arm approach to guide clients who are standing. S5 indicated S1 and C1 attend another location and an internal investigation was conducted. Four out of four client (S2 – C5) interviews indicated staff has never handled them in rough manner nor pushed them.
Regarding the allegation, “Staff handled client in care in a rough manner, based on record review and interviews, the Department found no evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, as a result, the allegation is Unsubstantiated.
An exit interview was conducted and a copy of this report was provided to with Program Director Marisabel Silva.
