The inspector’s account

On 09/18/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Rodrigo Mateo as the purpose of the visit was explained. The facility is licensed to serve (4) developmentally disabled adults ages 18 through 59 and which (1) may be non ambulatory. The clients are Harbor Regional Center consumers. Facility fees are current.

The facility is a single-story structure located in a residential neighborhood. It consists of the following: (4) client's rooms, two (2) common bathrooms, a living area, dining area, laundry area, kitchen, an outside shaded patio area, and a detached garage that is used for storage and houses emergency food supply and water. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the client's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to clients. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Fire extinguisher was charged, and smoke detectors and carbon monoxide were operable. Last fire drill was conducted on 08/07/25. A land line and internet service were observed. There were no bodies of water, obstructions, or fire arms on the premises.

LPA conducted a records review of 4 staff records, 4 client records, 4 P&I ledgers, and 4 medication administration records, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked.

Exit interview conducted, and a copy of this report was provided.
