Home Care Organization Evaluation Report

Home Care Services Bureau Enforcement Analyst (EA) Ryan Chan arrived at the business office of Day and Nite Homecare Services on 9/16/25 for a biennial inspection, EA met with licensee Augustina Urubusi. The proper posting of business hours and license was observed. The proof of insurance records were reviewed.

Licensee stated she does not have any home care aides or clients currently. EA provided guidance on office hours vs operating hours and provided form HCS 200 so licensee can update her office hours. EA also provided form HCS 500 (Training log) and Fast Facts for non-medical services.

EA Chan concluded the visit with an exit interview and provided a copy of this report to the licensee.
