Facility Evaluation Report

On 10/01/25 Licensing Program Analysts (LPA) Lizeth Villegas conducted an unannounced annual inspection. LPA met with Director Darcy Farias as the purpose of today's visit was explained. The day program is licensed for a capacity of 30 non-ambulatory adults ages 18-59. Clients are linked to the Harbor Regional center, the staff to client ratio is (1) staff to (3) clients. Current census is 44, 28 clients in the PM session and 16 clients in the PM session. Facility fees are current.

The day program is a single story building and consist of the following: a gym room, a garden, a computer lab, a style room, an art room, game room, a rest area, laundry room, (3) bathrooms, break room/kitchen, (2) administrative offices, and a gated outside patio. The Facility walls and floors were in good condition, adequate lighting observed. Chemicals and sharps were properly locked. The restrooms were clean and within Title 22 regulations. Water measured between 105 F.- 120F.. Clients bring their own meals and program provides snacks only. LPA observed lockers for clients to store belongings. A Land line observed, smoke alarms are hardwired and operational, carbon monoxide detector observed at main entrance. The last fire drill was conducted 09/24/25, (2) Fire extinguishers were properly charged, and the first aid kit was available and fully stocked. There are no bodies of water not fire arms on the premises, walkways throughout the day program and all exits were clear of hazards and debris.

As a part of today's inspection LPA reviewed (4) client records, and (4) staff records, no discrepancies observed. Currently (1) client who receives medication assistance at the day program, medications are locked and stored in the Directors office.
