Facility Evaluation Report
On 05/15/2025 at 12:30 pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Martha Flores Santana, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (4) non-ambulatory adults ages 18 through 59. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. One (1) client has a Restricted Health Care Condition, one (1) client is utilizing postural supports and none of the client are using protective devices.
The facility has a active administrator certificate #7015931735 for Martha Flores Santana valid date 05/11/2024 - 05/10/2026. The facility fees are current.The facility has liability insurance with AmGuard Insurance Company with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #C1GP506128) effective date 04/24/2025 - 04/26/2026. The facility fees are current.
The facility is a single story home located in a residential neighborhood. The property consists of the following: four (4) bedrooms, three (3) bathrooms, a kitchen, a dining room, a laundry area, a office area, an attached garage with an additional supplies and an outdoor shaded area in the backyard.
Between the hours of 1:18pm - 2:05pm, LPA conducted a records review of (4) client records, (4) staff records, (4) Client Medication Administration Records, (4) clients Personal & Incidental Records, and the facility disaster plan. All client & staff records were complete. The facility disaster plan was current (last conducted on 04/10/2025) and in compliance with Title 22 at the time of visit. LPA did not observed any discrepancies at the time of visit.
