The inspector’s account

This complaint alleged that the facility did not mitigate the spread of bed bugs. LPA Calderon toured the facility with S1. LPA Calderon noted staff cleaning floors, bathrooms and rooms. LPA Calderon inspected the dining room and kitchen area and noted that the kitchen and dining rooms were very clean. LPA Calderon inspected apartment 7, 14, 19, 22, 23, 24. All apartments were cleaned and did not have bed bugs. Record review indicates the following: Reviewed pest control reports for April, May and June 2025. Reports indicate that pest control sprayed all common areas and 5 units for all pest to include bed bugs. Report do not indicate any bed bug issues. Reviewed email between resident conservator and the facility administrator. The email suggest that the administrator took the complaint from the family member of bed bugs as a serious issue. The email suggest that the facility cleaned the apartment. The email suggest that the administrator offered to move R1 while the apartment was professionally cleaned. Interviews indicate the following: S1 indicates that he worked with the resident family to mitigate the bed bug claim. S1 indicates that he moved the 3 other residents out of the apartment for 3 days while his staff cleaned and sprayed for bed bugs. S1 indicates that R1 would not move which is supported by the email received. S1 indicates that he mitigated the bed bug claim when it happened. S2 indicates that his staff sprays for pest weekly. S2 indicates that 3 residents were moved out of the apartment for 3 days while staff sprayed and cleaned the apartment. S2 indicates that R1 refused to be moved to a new apartment while the prior apartment was being cleaned. S2 indicates that the facility plans to close the apartment for a week and have pest control spray and his staff clean and heat the room to kill any bed bugs. S3-S4 deny the allegation of any bed bug issues. R1 indicates that there were bed bugs in the apartment but R1 has not seen any new bed bugs since staff sprayed for bed bugs. R2-R4 who are roommates with R1 indicate there is no bed bug issues. R2-R4 indicate that the facility did move them out of the apartment for 3 days and the apartment was cleaned. R5-R8 indicate there are no bed bugs in the facility or in their apartments.

Based on records review observations and interviews, the preponderance of evidence standard has not been met; therefore, the allegation of “staff did not properly mitigate the spread of bed bugs at the facility” is found to be UNSUBSTANTIATED.

No deficiencies cited during today's visit.

An exit interview was conducted, and a copy of the Complaint Report was provided to the Administrator Ibrahim Zayat S1.
