Complaint Investigation Report
The investigation revealed the following:
Staff do not provide residents adequate food service.
It was alleged that there is a food shortage and management is not allocating enough funds for proper meals resulting in clients not receiving proper nutrition and multiple incidents of food poisoning.
On 11/14/2025 between the hours of 8:39am - 9:01am, LPA interviewed A1 who denied the allegation and explained Rainbow Produce delivers fruits and vegetables every week. A1 also stated the facility goes grocery shopping every Monday and Friday for meats such as beef, chicken, bacon, porkchops, hot dogs, burger patties, stew beef, and diary products (milk, cheese, yogurt, etc.)
Between 9:34am - 10:52am, LPA interviewed 7 staff: 5 of 7 staff denied the allegation and stated the facility ensure there is enough balanced and nutritious meals for all the clients. 2 of 7 staff who had no knowledge of the allegation stated their job duties is solely to provide housekeeping.
Between 9:04am- 9:31am, LPA interviewed 7 clients: 6 out of 7 clients denied the allegation and stated the facility serves enough food for breakfast, lunch and dinner with fruits and vegetables being incorporated daily in the clients meals with the option for clients to receive more food (such as seconds) upon request. 1 out 7 clients confirmed the allegation and stated the food served appears to look like larger portions but its actually very little food.
On 11/15/2025 between the hours of 11:04am - 11:09am, LPA conducted a tour of the facility kitchen and pantry and observed the following: the refrigerators and freezers are fully stocked with bread, shrimp (in fridge 3), vegetables (in fridge 4), turkey, eggs, lettuce, cheese, tortillas (in fridge 6), hot dogs, milk (in fridge 7), chicken, porkchops, ground beef, buns for hot dogs and burgers (fridge 8) and chicken (in fridge 9). Also LPA observed emergency prepareness and canned good as well.
On 11/15/2025, between the hours of 11:38am - 11:52am, LPA conducted a records review and observed the following: Statements from Rainbow Produce LLC with the sales invoices dated for 02/28/2025, 04/30/2025, 06/30/2025 and 08/31/2025) .The facility purchased groceries from Smart & Finals for the clients on 10/17/2025 and 10/25/2025 , Also LPA observed a facility menu for the month of November for the Week 1, Week 2 and Week 3 with a variety of different food options for breakfast, lunch and dinner. LPA did not observe any records to support the facility is not providing adequate food services.
Based on the information gathered, interviews, and record reviews, there is not enough evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is unsubstantiated.
