Facility Evaluation Report

On March 24, 2025, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced case management visit to facility. LPA Lee met with Voneric Johnson, Program director and explained the purpose the visit.

The purpose of today’s visit is to ensure that the Decision and Order of Immediate Exclusion from facility for staff Yolanda Lucia Flores was adhered to.

On March 17, 2025, LPA sent administrator a copy of the Decision and order via email (Administrator confirmed receipt of the Decision and Order).

During the visit, LPA found that above-name excluded individual is not on facility roster nor on the premises at time of visit. Program Director informed LPA that the above-named excluded individual has never worked at the facility.

There were no deficiencies cited during today’s visit.

An exit interview was conducted with Program Director, Voneric Johnson and a copy of this report in addition to a copy of Government Code 11522 were issued, informing the licensee that an excluded person may petition for reinstatement to the Department one year after the effective date of the exclusion order.
