On 10/20/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced case management deficiencies. The purpose of the visit is to issue a citation observed during the complaint investigation - Control 11-AS-20251015105817.
During the complaint investigation, LPA learned that there was no documented care plan for the use of a catheter.
Deficiency cited under California Code of Regulation Title 22 Division 6 Chapter 8 are being cited on the attached LIC 809-D.
