The inspector’s account

On 08/15/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the new CARE Inspection Tool. LPA met with Administrator Nestor Zaragoza as the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory only developmentally disabled adults ages 18 through 59. All (4) clients are Harbor Regional Center consumers. Facility fees are current, surety bond active (RLI #RSB0748497 Exp: 06/01/2029).

The facility is a one story structure located in a residential neighborhood. It consists of the following:(4) client's rooms, (2) staff bedrooms, (2) common bathrooms, living area, dining area, kitchen, patio area and an attached garage that serves as an administrative office and laundry room. The facility has a working land line and internet service throughout the facility. Beds and bedding supplies were in good condition, adequate lighting is provided, storage for client personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The water temperature measured 105-120 F.. Toxins, and sharps are stored and not accessible to clients. The kitchen was inspected and there is sufficient perishable and non-perishable food available. Fire extinguishers were charged, smoke detectors and carbon monoxide were operable. Fire drill conducted on 05/08/25. There are no bodies of water or obstructions on the premises. Exits/ Walkways around the facility were free of debris and hazards.

LPA conducted review of 3 staff file, 4 client files, 4 P&1 ledger, and 4 Medication Administration Records (MAR), no discrepancies observed.

During today’s visit no discrepancies were cited.
