Facility Evaluation Report

On 10/21/2025 at 1:11pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Maria Andrade, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for a capacity of 4 developmentally adults ages 18 through 59 and approved for 4 non-ambulatory. Currently, the home has (4) ambulatory clients. The clients are Harbor Regional Center clients. None the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a current administrator certificate (#7009038735) for Maribel Andrade valid from 04/04/2024- 04/03/2026. The facility fees are current.The facility has liability insurance with Kinsale Insurance with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #0100308895-1) effective date 06/24/2025 - 06/24/2026.

The facility is a one story home located in a residential neighborhood. The property consists of the following: 4 client bedrooms, 2 common bathrooms, staff office, living room, kitchen, dining room, attached garage, an outdoor shaded area and backyard which houses the washer and dryer.

Between the hours of 1:55pm -2:45pm, LPA conducted a records review of (4) client records, (4) staff records, (3) clients Personal & Incidental Records, (4) Client Medication Administration Records and did observe a discrepancy at the time of visit. All client & Staff records were complete. LPA reviewed the facility disaster plan. The facility disaster plan was current and in compliance with Title 22 at the time of visit.
