Facility Evaluation Report

On 12/14/2023 at 11:30 AM Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced Annual required visit to Ambitions Long Beach Day Program Facility with a primary focus on infection control measures. LPA Calderon was allowed entry into the facility by Manager Mark Arellano. Manager Arellano asked infection control questions and took LPA Calderon temperature prior to entrance into the facility and the purpose of today’s visit was explained. The facility is licensed to serve 60 adult clients with behavioral issues.

There are currently 10 adult clients with behavioral issues in care. The facility is a one-story commercial structure with (6) rooms and (2) bathrooms, activity room. kitchen and work out room, activity rooms, computer room and staff offices.
