Facility Evaluation Report
On 07/14/2025 at 8:10 AM, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Latashia Hammond (Administrator) and the purpose of today’s visit was explained.
The facility is licensed to operate for (5) ambulatory developmentally disabled adults ages 18 through 59. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the client have Restricted Health Care Condition nor are not utilizing postural supports and protective devices.
The facility has a current administrator certificate (#7012434735) for Latashia Hammond valid from 11/14/2024 - 11/13/2026. The facility fees are current. The facility has liability insurance with Great American Assurance Company (NAIC# 26344) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #PAC344830025) effective date 09/29/2024 - 09/29/2025. The facility fees are current.
The facility is a single story home located in a residential neighborhood. The property consists of the following: four (4) bedrooms, two (2) bathrooms, a kitchen, a dining room with a den, a laundry room, a office area, an attached garage with an additional supplies and an outdoor shaded area in the backyard.
