Facility Evaluation Report
On July 23, 2026 Licensing Program Analyst (LPA) Mario Leon arrived at program to conduct unannounced annual using the CARES inspection tool. California Department of Social Services (CDSS) met with Program Manager, Havannah Ulrich (S1), and the purpose of today’s visit was explained. The facility is licensed to serve 30 adult clients. The program hours are from 9:00AM - 3:00pm.
The facility is a single-story building, located in a business district in Bixby Knolls. The building consists of the following: 3 classrooms, 1 therapy/relaxation room, 1 observation room attached to the therapy room equipped with one-way glass, 1 staff office, 1 community/lobby room, 1 kitchen, 1 changing room equipped with changing table, 1 staff break room, 1 program manager office and 3 restrooms for client and staff use.
Physical Plant CDSS and S1 toured the facility inside and outside. CDSS observed that there were no bodies of water on the premises. The facility was clean and sanitary with no hazards or obstructions. All rooms were equipped with adequate lighting and plenty of space for activities. The Bathrooms were operational, water temperature measured at 109 °F (degrees Fahrenheit), within title 22 regulations. A comfortable temperature of 72 °F was maintained in the facility.
