Facility Evaluation Report
On July 30, 2025 Licensing Program Analyst (LPA) Deborah Lee arrived at program to conduct unannounced annual using the CARES inspection tool. LPA met with Administrator, Havanna Ulrich and the purpose of today’s visit was explained. The facility is licensed to serve 30 adult clients. The program hours are from 9am-3pm.
The facility is a single-story building, located in a business district in Bixby Knolls. The building consists of the following: 3 classrooms, 1 therapy/relaxation room, 1 observation room attached to the therapy room equipped with one-way glass, 1 staff office, 1 community/lobby room, 1 kitchen, 1 changing room equipped with changing table, 1 staff break room, 1 program manager office and 3 restrooms for client and staff use.
Physical Plant LPA and Administrator Havanna toured the facility inside and outside. LPA observed that there were no bodies of water on the premises. The facility was clean and sanitary with no hazards or obstructions. All rooms were equipped with adequate lighting and plenty of space for activities. The Bathrooms were operational with water temperature measured between 105 and 120 degrees F. A comfortable temperature of 75 degrees F. was maintained in the facility.
