Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff and the purpose of the visit was explained. Administrator Ana Navarro arrived shortly after.
The facility is licensed to serve four (4) developmentally disabled adults ages 18-59 years old. The facility is approved for (1) non-ambulatory client. The clients receive services from Harbor Regional.
The facility is a single story home located in Lomita. The home consists of four (4) client bedrooms, one (1) staff room, two (2) bathrooms, living room, dining room, kitchen, an office, a rear patio and a garage.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
LPA conducted a record review of four (4) client files and five (5) staff files. All files were complete and observed to contain the required documents. Medications are centrally stored in the office. LPA reviewed four (4) client Medication Administration Records, medications are documented properly and given as prescribed.
The facility disaster plan was not the current revision at the time of visit, Administrator agreed to update the document. Technical violation issued. Carbon monoxide detector was observed in the dining area, was tested and operational. Smoke detectors were observed in client bedrooms, were tested and are operational. Fire extinguisher was observed located in the laundry area, last service 05/06/26. Emergency drills are conducted monthly, last documented emergency drill was conducted July 4, 2026.
