Facility Evaluation Report
On June 18, 2025, Licensing Program Analyst (LPA) Ernand Dabuet conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the house manager Lilia Green. LPA explained the purpose of today’s visit. The facility is approved for developmentally disabled adults ages 18-59 years old. The facility is approved for (1) non-ambulatory client. The clients are Harbor Regional Center consumers.
The facility is a single story structure with five client's rooms and two bathrooms, living room, dining room, kitchen, an office, a rear patio and a garage.
An audit of client's service records for client's #1-#4 (C1-C4) is accurate and complete. Several working landline phones are available on-site.
Due to unforeseen situations with clients in the home, the inspection must be terminated early. Due to time constraints, an additional visit is needed for the annual continuation inspection.
No deficiencies cited during this inspection visit.
An exit interview with Lilia Green and a copy of the report is provided.
