Facility Evaluation Report
On 06/16/2023, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual required visit using the new CARE Inspection Tool. LPA met with Ana Navarro, Administrator, and explained the purpose of today’s visit. The facility is licensed to operate for four (4) developmentally disabled clients between the ages of 18 through 59 of which one (1) can be a non-ambulatory client. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. The facilities annual fees are past due with a balance of $2,497.00.
The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) client rooms, one (1) staff room, one (1) office space, two (2) bathrooms with one (1) of those bathrooms being in the staff room, a living area, a dining area, and kitchen. There is an outside patio area with ample seating and an umbrella used for shade. The washer and dryer are located in the hallway near the rear rooms. The garage is attached with access from the front of the garage and a side door directly inside the front door. Garage is used for storage only.
LPA conducted a records review of (4) client records, (4) staff records, (4) clients Personal & Incidental Records and reviewed the facility disaster plan. All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) Client Medication Administration Records and did not observe any discrepancies at the time of visit.
