Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff, and the purpose of today’s visit was explained. Administrator Thomas Signey after to assist with file review.

The facility is licensed to operate for (6) ambulatory developmentally disabled adults ages 18 through 59. Currently, the home has two (2) clients. The clients receive services from Harbor Regional Center.

The facility is a single story home located in a residential neighborhood of Lomita. The home consists of the following: three (3) client bedrooms, two (2) bathrooms, staff office, living room, kitchen, dining area, attached garage, laundry room and an outdoor shaded area.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

LPA conducted a record review of two (2) client files and three (3) staff files. All files were observed to contain the required documentation staff. Medications are centrally stored in a cabinet in the hallway. LPA reviewed two (2) client Medication Administration Records. Medications are documented properly and given as prescribed.
