The inspector’s account

On 12/12/25 Licensing Program Analyst (LPA) Mario Leon conducted an unannounced required annual visit at the facility. LPA was met by staff one, Brenda Peralta - Program Director (S1), and the purpose of the visit was explained. The facility is licensed for a capacity of 18. S1 stated that the facility has fifty-nine (59) clients enrolled, with seventeen (17) clients currently indoors, at the program, during today's visit. The staff to client ratio is (1) one staff to (3) three clients. The last fire & disaster drill was conducted 08/08/25 and a Disaster Plan is on file. The last inspection held by the fire department was on 05/02/25. The program does not provide transportation.

As a part of today's inspection LPA reviewed four (4) client records, (4) staff records, and two (2) Medication Administration Records and inspected the physical plant. The day program is located in one large building consisting of the following: parking lot (drop off/pick up area), lobby area with a receptionist area in front, 7 rooms (spa room, quiet room, art room, game room, equipment room, music/leisure room and computer room, kitchen, storage area, supply room, 2 offices (1 case manager office, 1 program manager office) 2 bathrooms (with 1 restroom with a locked storage room), staff/isolation area, outside shaded backyard along with a garden area.

At 12:24pm, LPA conducted a tour with S1 of the physical plant and observed the following: walls and floors were in good condition, adequate lighting and two (2) fire extinguishers were properly charged, last checked on 12/01/25. LPA observed plenty of storage space and chemicals were properly locked. The restrooms are clean and within Title 22 regulations. The kitchen is clean and a refrigerator is available for client use. The day program does not provide lunch nor snacks to clients. The first aid kit was available and fully stocked; medications were locked and inaccessible to clients. Walkways throughout the day program and all exits were clear of hazards and debris.

There have been zero (0) deficiencies cited during today's visit. An exit interview was held with Brenda Peralta and a copy of this report has been provided.
