Regarding Allegation #1 : Staff pushed C1 at a day program: It is being alleged that staff pushed C1 at the park on 05/03/2024. Toured the facility and LPA noted staff and client interactions that met the needs of the client. Reviewed Harbor Regional Center investigation (date 05/22/2024) which indicates “regarding the claims in the SIR that C1 was forced to the ground by S2 from this investigation it appears substantiated that an interaction occurred between C1 and S2 that resulted in C1 falling to the ground”. Reviewed staff training in-service (date 05/28/2024): Topic appropriate methods to interacting with participants. Reviewed Torrance Memorial records (date 05/08/2024) which indicate that C1 indicated to doctor that C1 was tackled and landed on right hip. X-rays were done and no abnormality or fracture was noted.

The interviews indicate: S1 indicates that S1 was not informed that C1 was injured in the park. S1 indicates that S1 was informed that C1 tripped and fell to the ground and injured C1 right hip. C1 indicates that S2 was playing around with C1 who stepped in a hold and fell to the ground. S2 indicates that S2 was picking up baseballs and C1 was grabbing the baseballs. S2 indicates that C1 step into a hole and lost C1 balance and fell to the ground. S2 indicates that C1 was taken back to the day program and given an ice bag for C1 pain. S2 indicates that S2 informed S1 and an incident report was generated. S2 indicates that S2 was given training on how to deal with clients and S2 indicates that if S2 would not have played around with C1 would have not been injured. C1 indicates that C1 was at the park and S2 picked up C1 and swung C1 around and C1 fell to the ground and injured C1 right hip. 7 out of 8 clients indicate that staff has never pushed them or injured their person.
