The inspector’s account

On 03/16/25, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with DSP, Mariandre Arreaga, and the purpose of the visit was explained. Administrator, Yadira Martinez, joined LPA for the visit, shortly after. The facility is licensed to operate for six (6) developmentally disabled adults ages 18 through 59. Currently, the home has (4) clients are all non-ambulatory. There is an approved exception to utilize postural supports (half bedrails) and/or protective devices (wheelchair seatbelt). Facility’s annual fees are current.

The facility is a single story house located in a residential neighborhood and consists of the following: Four (4) bedrooms, three (3) bathrooms, living room, kitchen, dining room, office area, laundry room, activities room, an attached garage with an additional refrigerator and freezer and two (2) outdoor shaded areas.

LPA Gonzalez conducted a review of (3) client records, (3) staff records, (3) client Medication Administration Records and reviewed the facility disaster plan. LPA observed the reviewed records to be complete, and in order. The last fire drill was conducted on 01/24/2026.

LPA Gonzalez and Mariandre Arreaga toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit.
