Facility Evaluation Report
On 03/05/2025 at 8:28 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Yadira Martinez, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for developmentally disabled (DD) adults ages 18 through 59. Currently, the home has (4) clients are all non-ambulatory. The (3) clients are with Westside Regional Center and (1) client is with Harbor Regional Center. As of 01/14/2025 the department approved an exception approval for the clients to utilizing postural supports (half bedrails) and or protective devices (wheelchair sealtbelt). The annual fees $454 is due on 03/27/2025 and LPA provided pin# 812163 with the option to make a payment online. The liability insurance is with Philadelphia Ind.Ins.Co (NAIC #18058) each occurrence is $1,000,000 and general aggregate is $3,000,000 which is valid from 05/21/2024 - 05/21/2025.
The facility is one story house located in a residential neighborhood. The property consists of the following: 4 client bedrooms, 3 common bathrooms, staff station, living room, kitchen, dining room, a laundry room with additional linen, garage with an additional refrigerator and freezer and two (2) outdoor shaded area.
Between the hours of 10:00 am - 10:45am LPA conducted a records review of (4) client records, (4) staff records, (3) clients Personal & Incidental Records and reviewed the facility disaster plan (last fire drill conducted on 12/18/2024 and the last fire drill was conducted on 02/22/2025) . All client & Staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. Between the hours of 9:20am - 9:35 am, LPA reviewed (4) Client Medication Administration Records and did not observed any discrepancies at the time of visit.
