Facility Evaluation Report
On 4/22/26, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced required annual visit using the CARE Inspection Tool. LPA met with staff, Mark Santos, and explained the purpose of the visit. LPA was granted access to the facility. Administrator, Adriana Galeano later joined LPA for the visit. The facility is licensed to serve six (6) developmentally disabled adults ages 18 thru 59 years old.
LPA Gonzalez and Adriana toured the inside and outside grounds of the facility. The facility is a single story home located in a residential neighborhood. The facility consists of four (4) bedrooms, two (2) bathrooms, living room, kitchen, dining area, and patio.
All client bedrooms were inspected. LPA observed the bedrooms to have the required furniture. Beds had the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillow. LPA observed ample lighting in all the bedrooms and hallway. The bathrooms were inspected and found to be clean and operational. Water temperature properly measured between 105.0 F and 120.0 F.
LPA Gonzalez and Adriana toured the kitchen and found it to be clean and sanitary. All appliances were in good working order. Sharps and knives were locked and inaccessible to clients, stored in a kitchen drawer. Medications were locked and stored in the cabinet located in the dining room. LPA observed an ample supply of perishable and non-perishable foods was maintained.
LPA Gonzalez and Adriana walked through all common areas. In the living room, kitchen, dining room there is ample seating and space for all residents. All rooms and walkways were clean, and clear of obstructions and hazards.
