Facility Evaluation Report
On 04/05/2024 at 8:00 am Licensing Program Analyst (LPA) David España conducted an unannounced Required-1-year annual visit. Upon arrival at the facility, LPA España conducted a risk assessment at the front door. Based on the assessment, the facility is clear of Covid-19 infection (No COVID-19 cases). LPA verified that the facility has an approved mitigation plan report. LPA was granted access and allowed to enter the facility to conduct the inspection. On today's visit LPA met with Administrator, Adriana Galeano explained the purpose of today’s visit. The facility is licensed for six (6) non-ambulatory clients. Currently the facility has 3 clients. The facility is a single-story home located in a residential neighborhood. The facility consists of four (4) bedrooms, two (2) full bathrooms, living room, dining room, office area, laundry area, kitchen, and detached garage.
LPA and administrator toured the inside and outside grounds of the facility. In the backyard, LPA observed a covered patio with table and chairs. All walkways and ramps have secured handrails. On the back of the property is an additional patio with a picnic table and two (2) sheds used for storage. In Two (2) shed is an ample supply of incontinence product for clients, and in the second is an additional supply of cleaning supplies, sanitizer, and toiletries. Continued on LIC809C
