Facility Evaluation Report

On 2/11/26, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA met with the Administrator, Meera Gopy and explained the purpose of today’s visit. The purpose of today’s visit was to conduct the required annual inspection, using the new Care Tool. The facility is licensed approved for (4) ambulatory developmentally disabled clients (age 18-59). The census is currently 0.

Lumiere Villa, Inc. is a one-story structure located in a residential neighborhood. This facility is currently undergoing renovation. All rooms are empty and repainted. The bathroom is currently gutted. The facility consists of (2) client bedrooms, (2) bathrooms, living room, kitchen, dining area, detached garage, washer and dryer is located outdoors and there is an outdoor shaded area. There is a pool that is locked with a 6ft fence surrounding it. The walls do not have required posting as the walls are being painted and there are no clients on roster for this facility.

LPA conducted a records review of (3) staff records. All staff records were complete. LPA Shirley and Meera walked throughout the facility and all common areas such as the living room, dining room , and bathrooms. LPA could not check the water temperature as the water service was turned off.

LPA observed that there is ample space for all clients. All walkways were clean, and clear of obstructions and hazards. All renovation materials are being stored in the backyard at this time. Smoke/ Carbon Monoxide detectors were operable. There is (1) fully charged fire extinguisher.

No deficiencies were cited during this inspection visit.

An exit interview was conducted, and a copy of this report was provided to the Administrator, Lilowtee (Meera) Gopy.
