Facility Evaluation Report
On 6/18/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA was met by the Assistant Administrator, Sarita Wellington and explained the purpose of today’s visit and was granted entry. The facility has a census of 4.
The facility is a one-story structure located in a residential neighborhood. The facility consists of (4) client bedrooms, (1) staff room (1) full bathroom and 3 ¼ size bathrooms, kitchen, dining area, living room, garage, back yard, and shaded area on the front porch. Facility maintains all required posting throughout the facility.
LPA Shirley reviewed all client’s facility files and observed all required documents. LPA Shirley checked client’s medications. The medications were locked and stored in a cabinet in the hallway and inaccessible to residents. LPA Shirley checked clients MAR against the labeled medications. LPA found there to be a fully stocked first aid kit with an up-to-date Red Cross manual. LPA Shirley reviewed the Surety Bond.
LPA Shirley, and Sarita walked through the kitchen and all appliances were in good working order. Knives were locked and stored in a cabinet in the kitchen inaccessible to persons in care. LPA observed a 3-day supply of perishable and a 7-day supply of nonperishable foods. This facility has 1 fire extinguisher that is inspected yearly and located in the kitchen. The water temperature measured 103.7 degrees Fahrenheit.
