Facility Evaluation Report
On 02/20/26 at 10:00 am Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced annual visit for this facility. There was no answer at the door and LPA called the Administrator, Scholastica Anusionwu. Administrator stated that there are currently no clients living in the facility, it was explained that the annual inspection still needs to take place. Administrator arrived shortly after and entrance interview was conducted.
Infection Control and Emergency Preparedness Plan: LPA reviewed facility Infection Control Plan and Emergency Preparedness Plan to make sure protocols are being reviewed and/or updated. Plans were discussed and updated.
A tour of the physical plant was conducted at 11:30 am with Administrator. The facility has four (4) bedrooms and two (2) bathrooms. It is currently not occupied by clients as they are still waiting for the final process from North Los Angeles Regional Center. The tool kit was not used as there are no clients or staff.
Kitchen: LPA observed the kitchen area to have a refrigerator, microwave oven and sink. There will be an adequate supply of two (2) days perishable food once clients are in the home. There was a seven (7) day supply of nonperishable food and dining ware to accommodate a maximum capacity of four (4). Knives are locked in a kitchen cabinet within a locked box. The facility has one fire extinguisher that was last serviced on 11/03/2025 located in the kitchen.
