Facility Evaluation Report
On 07/21/2023 at 9:15 a.m. Licensing Program Analyst (LPA) Evelin Rios arrived at the facility above to conducted a Case Management - Legal/Non-compliance visit of the facility to ensure facility compliance. LPA rang the door bell and nobody answered. LPA called number for facility and number listed for Licensee and left a voicemail. LPA emailed Licensee and Administrator. Administrator Jessica Hernandez called LPA and informed LPA they would meet LPA at the facility. Administrator met LPA shortly after and LPA explained the reason for the visit. On January 17, 2023, a Non-Compliance Conference (NCC) was held at the Woodland Hills Regional office. As a result of that NCC, the facility was placed on a two-year compliance plan.
At approximately 10:00 a.m., LPA and staff took a tour of the physical plant. LPA observed smoke/carbon monoxide detectors through out the facility that are interconnected and hardwired. Smoke/carbon detectors were tested at 10:07 a.m. and functioning properly. The fire extinguisher is in the pantry was observed fully charged with last service date 10/28/2022.
Kitchen: The kitchen appliances and fixtures were functional. LPA observed a two day supply of perishable and a seven day supply of non-perishable food at the facility; properly stored. LPA observed sharps or knives in a locked box properly stored in a locked kitchen cabinet inaccessible to clients in care. Properly labeled medications were locked in one of the kitchen cabinets.
Bedrooms: There were four (4) bedrooms designated for resident use. All the bedrooms were properly furnished with appropriate beddings, linens and sufficient lighting. LPA observed extra linens stored in a hallway closet.
Bathrooms: There are two (2) bathrooms one of which is designated for resident use. Bathroom was properly supplied with paper towels, hand soap and had functional fixtures. Hot water temperature was measured at 117.8 degrees Fahrenheit. LPA observed overflow of toiletries in the bathroom for client use. (LIC809 Continued on next page)
