The inspector’s account

At 10:30 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced annual inspection in conjunction with complaint visit #31-AS-20250417162535. The LPA contacted the Administrator via telephone and explained the purpose of the visit. The Administrator initially informed the LPA that they were unable to come to the facility due to a doctor’s appointment; however, at 11:15 AM, the Administrator contacted the LPA and stated that he would be coming to the facility. The Administrator arrived at 12:53 PM.

Upon arrival, the LPA toured the facility. At the time of the visit, there were no clients in care, and the facility was observed to be unoccupied as of May 2025.

The Administrator stated that the facility is currently working with the Regional Center to reopen and resume client admissions. The Administrator further informed the LPA that once a client is admitted, the Administrator will notify the Community Care Licensing Division (CCLD) accordingly.

The LPA conducted a physical plant inspection. The facility was observed to be in clean and in good condition with no immediate health or safety concerns noted. The LPA observed newly purchased linens, bedding, towels, hampers, and a cart for client hygiene supplies.

No clients were present during the visit; therefore, no interviews were conducted.

An exit interview was conducted with the Administrator. A copy of the report was reviewed and provided.
