Facility Evaluation Report

At 10:30 AM, Licensing Program Analysts (LPAs) Huma Rahimi and Evelin Rios attempted to conduct an unannounced visit to the above facility. LPA Rios contacted the Administrator to grant access to the facility to deliver complaint report control # 31-AS-20250515092725 and LPA Rahimi to deliver an amended report of the complaint control #31-AS-20240924102410. The Administrator stated that they cannot meet at the facility to sign today's report. The Administrator attempted to send a designee to sign; however, the designee was also not able to come. The Administrator informed LPAs that he is able to stop by at the Woodland Hills Regional Office to sign both reports and receive a copy before 3:00 PM, on 9/10/2025.

At 3:00 PM, the Administrator arrived at the Regional Office to sign the reports; however, upon reading the reports, the Administrator refused to sign this report and the amended report for the complaint control # #31-AS-20240924102410.

Exit interview conducted and copy of this report will be mailed to the Administrator/Licensee.
