Facility Evaluation Report
At 10:15 AM, Licensing Program Analysts (LPAs), Huma Rahimi, Evelin Rios along with North Los Angeles Regional Center Community Services Specialist Andrea Messina, conducted an unannounced annual inspection in conjunction with the complaint # 31-AS-20250515092725 visit. LPAs called the Administrator Tyrone Quals to let them know the reason for the visit and the Administrator stated that they would not be able to meet with LPAs. Administrator informed LPAs the house manager would arrive at facility and sign for today's report. The team gained access to the facility at 12:00 PM.
At 12:15 PM, LPAs and the house manager conducted a tour of the facility. The house manager stated the facility has no clients and is still vendorized by the North Los Angeles Regional Center.
Common Areas: LPAs toured all common areas of the facility. LPAs observed common areas to be clean and furniture to be in a good repair. The facility maintains a comfortable temperature at 78 degrees F. LPAs observed the fire extinguisher to be full and last serviced on 01/30/25.
Client room s : LPAs observed client’s rooms to have appropriate bedding sheets, pillowcase, mattress pad, and blankets, which are in good condition. There is at least one chair, night stand and sufficient lighting for each client.
Bathrooms: At 12:25 pm, LPAs observed the bathrooms to have a non-skid matts and appropriate grab bars in each bathroom. The hot water measured at 116.2 degrees F,
