Facility Evaluation Report

On 06/02/2023 Licensing Program Analyst (LPA) Evelin Rios conducted an announced Case Management - Deficiencies visit in conjunction with complaint control #31-AS-20230601142516,. During physical plant tour at 1:44 p.m. LPA observed tide pods, laundry detergent in R2's private bathroom. LPA asked S1 why detergent was in a resident's bathroom. According to S1 it had been left there by another resident. Licensee then entered the bathroom and observed the tide pods, laundry detergent in the bathroom. S1 immediately removed the tide pods, laundry detergent from the bathroom.
