Facility Evaluation Report
On 07/15/2025 Licensing Program Analyst (LPA) Evelin Rios conducted a Case Management - Incident visit in conjunction with complaint control #31-AS-20250114101006 . LPA met with Maria Reyes and spoke to Licensee Asalia Wauls by telephone and explained the reason for the visit. Entrance interview conducted.
At approximately 2:10 p.m., LPA spoke with three (03) staff that informed LPA of client #1"s (C1's) refusal to allow staff to enter room to clean, throw out expired food and trash. LPA's interview with C1 revealed they are unwilling to allow staff to clean their room due to concerns items may go missing and they can not clean it themselves because they cannot lift items. LPA asked if C1 could just be an observer while staff clean. C1 started to raise their voice and vigorously point at LPA stating they would report LPA to their higher ups as they are not on C1's side. At 2:21 p.m., during physical plant tour LPA observed C1 bedroom with items covering the top of furniture and items appeared to be knocked over on the floor. LPA also observed crumbs on the floor. According to Maria staff last cleaned C1's bedroom 06/16/2025 and adult protective service (APS) has been out to the facility to investigate C1's alleged self neglect. Facility self reported an incident that occurred on 07/11/2025 reporting where C1 was observed by staff peeing on the carpet. LPA smelled a urine odor coming from C1's room. According the licensee they are working with C1, so that staff may enter their room without threats and conduct a deep clean. Licensee has also reached out to C1's Consumer Services Coordinator (CSC) from North Los Angeles County Regional Center for guidance. LPA will continue to monitor the progress facility is making towards cleaning and sanitizing C1's bedroom.
