On 09/15/25 at 10:00 am Licensing Program Analyst (LPA) Lorena Casillas arrived at facility to conduct an unannounced case management visit.
LPA was greeted and granted access by staff member Julissa Dubon Hall who contacted Administrator Monica Gory over the phone. Administrator informed LPA that they would not be able to join LPA for the visit and designated staff member to sign the report. LPA informed designee that today's case management visit was to amend annual report dated 09/03/25.
Staff member signed amendment, LPA printed and provided a copy to designee.
