Allegation #1 Staff speaks to resident in an inappropriate manner.

It is alleged that staff speak to clients in an inappropriate manner. Regarding this allegation it is reported that Client #1 (C1) was being interviewed by a State Council representative when C1 raised their voice towards Staff #1 (S1), S1 then began a verbal confrontation with C1. LPA interviewed Administrator and it was discovered that Administrator was on the phone with S1 at the time of the alleged verbal dispute and Administrator states that S1 did not raise their voice or argue with C1. S1 was responding to C1 and trying to de-escalate the situation. Interview with S1 revealed that S1 did not address C1 in a negative manner nor was C1 show any disrespect. S1 states that they were simply trying to de-escalate the situation and advising C1 that there was no need to raise their voice towards them. S1 states that C1 tends to address staff in a negative manner when there are other persons in the facility. LPA interviewed C1 and C1 stated that the information that was shared was misunderstood. C1 states that what was said and what was meant are different things and that C1 did not say that the staff speak to C1 in an inappropriate manner. C1 states that they like all the staff members and that all staff treat C1 with respect. LPA interviewed additional staff present and they corroborated that C1 is aggressive towards staff and all that staff do is try to de-escalate the situation and try to redirect C1. LPA interviewed clients C2 and C3 via phone and they state that staff are all nice to them and they do not have any problems with any of the staff members, nor have they witnessed staff speak to anyone in an inappropriate manner. During LPA’s record review it was discovered that C1 does have emotional outbursts however since being at this facility they have decreased as reported by staff in daily behavioral logs. Therefore, based on record reviews, observations and interviews this allegation is deemed Unsubstantiated at this time.

Allegation #2 Staff does not assist with clients showering needs.

It is alleged that staff do not assist with clients showering needs. Regarding this allegation it is reported that C1 is not able to shower correctly, and that staff do not assist C1 with showring needs. Interviews with staff and Administrator revealed that staff have offered numerous times to assist C1 so that C1 can shower properly, however C1 refuses the assistance claiming that there is no need for assistance. Administrator indicated that C1 is prompted to groom and staff follows in prompting but C1 at times does not comply. Interview with C1 revealed that C1 refuses the assistance offered by staff because C1 does not need the assistance.
