Facility Evaluation Report
At 11:45 a.m. on 08/07/2024, Licensing Program Analyst (LPA) Lorena Casillas conducted an unannounced case management visit. LPA met with the Program Coordinator Nicolas Williams and disclosed the reason for the visit.
Today’s case management visit is to follow up on an unusual incident previously reported on 08/02/24 by the facility in which Staff #1 (S1) allegedly used Clients #1 (C1)’s money to purchase lunch for C1 but instead S1 consumed the lunch themselves. The purpose of the visit is to obtain additional information regarding this incident.
LPA interviewed Program Coordinator, staff member #1 (S1) via telephone, staff member #2 (S2), staff member #3 (S3) and C1 from 12:00 pm to 1:15 pm.
On 8/2/24 a group of clients were on an outing to Burger King when S2 and S3 noticed that S1 purchased a meal for C1 with C1’s lunch money. S2 and S3 noticed that S1 gave C1 a few fries from the purchase along with C1’s home lunch, S1 was then witnessed consuming the meal that was purchased with C1’s money. S2 notified Program Coordinator of the situation and per Program Coordinator an investigation ensued. It was also noted that S1 at no point provided any money from their own wallet or pockets to purchase a meal. S1 was placed on an administrative leave until further notice to give time for an internal investigation. Program Coordinator confirmed that a purchase was made with C1’s money via a receipt that was provided to C1’s conservator, which reflects a meal that was purchased and change that was provided. Program Coordinator will hold a meeting with all staff to go over regulations and procedures regarding the handling of client funds and personal rights. The Program Coordinator reported the incident appropriately to Community Care Licensing, Adult Protective Services, and Long Term Care Ombudsman.
Since the Program Coordinator has taken the appropriate actions to ensure staff understand how to handle client’s money and personal rights, there are no deficiencies cited at this time.
