Facility Evaluation Report
Licensing Program Analyst (LPA) Lorena Casillas arrived at the facility at 09:30 am and was greeted by Licensee Kristy Jones. LPA explained the reason for the visit and entrance interview conducted. The facility has five (5) bedrooms and three (3) bathrooms. Four (4) rooms are to be used for clients and one (1) room is to be used as an office. Facility is fire cleared for four (4) ambulatory clients. The following was observed:
LPA and Licensee conducted a tour of the facility at 10:15 am. The following was observed:
There are no clients in the home as of today’s date. The tool kit will not be used during this visit.
Kitchen: At 10:20 am LPA observed there to be sufficient stock of two- perishable and seven-day non-perishables foods. Snacks and beverages are available for clients in the facility when they want. Frozen foods are properly wrapped and stored. Food storage and preparation areas are clean and inaccessible to pests. Knives are stored in a locked kitchen drawer.
Common Areas : LPA toured all common areas of the facility. LPA observed common areas to be clean and furniture to be in good repair. The facility maintains a comfortable temperature at 71˚F. LPA observed fire extinguishers to be last purchased on 08/14/24. Medication will be stored in a locked hallway closet inaccessible to clients. At 10:30 am, staff tested the carbon monoxide and smoke detectors, all were functioning properly. First aid kit had Licensing requirements and is located in the medicine cabinet.
