Facility Evaluation Report

LPA Spaeth arrived to the facility and was greeted by Administrator. LPA stated the purpose of the visit was to request resident documentation regarding an incident report received by CCL. Upon entering the building, LPA's temperature was recorded and LPA answered COVID questions.

LPA and Administrator toured the building between 10:30 am until 11:00 am. LPA did not observe any health or safety issues during the tour. LPA reviewed resident documents between 11:00 am until 11:25 am. At 11:45 am, LPA requested resident documents and the staff work schedule.

There are no deficiencies to report at this time. Exit interview conducted, appeal rights discussed, and a copy of the report was given to the Administrator.
