(Continued from LIC9099) At 12:00 pm., LPA initiated an interview with Staff #2 (S2) and Clients #4 (C4). At 1:03 p.m., LPA interviewed Staff #3(S3) by telephone. From 2:30 p.m. to 3:30 p.m., LPA reviewed the PRN or as needed medication for Client#1 (C1) and obtained copies of the facility's 15 Minute Medication check, PRN authorization form and daily notes for C1. LPA also interviewed Staff #4 (S4) and Staff #5 (S5), At 4:20 p.m., LPA interviewed C1.

Allegation: Staff does not ensure resident is accommodated with a longer mattress. It was alleged that a client needs a longer bed due to their leg pain and the facility has not made the accommodation. Based on interviews with the administrator and staff, C1 and C2 have broken their beds. Interview with the administrator revealed the facility constructed platforms with plywood and 2X4 pieces of wood to assist in holding up clients' mattresses. Furthermore the administrator revealed C1 is taller then their current bed which is a full size mattress. Interview with four (4) out five (5) staff corroborate C1 is tall but did not corroborate that C1 was taller in length to their bed and denied that C1 had made complaints to them about leg pain. LPA's interview with four (4) out of four (4) clients denied their beds were uncomfortable. LPA's interview with C1 revealed their bed is lopsided and may pop and break. LPA observed C1 lay on their bed and observed their feet hang off the bed slightly due to C1's height. Based on interviews and observation the allegation is Substantiated. Administrator stated the facility has purchased a queen size bed and will be putting it together for the client.

Deficiency issued, refer to 9099-D. Exit interview conducted. Copy of this report and appeal rights provided.
